What the Research Says About CBD and Hormones

Right now, there is no strong human research showing that CBD safely “balances” hormones. What exists is a small number of animal studies, lab-dish (in vitro) studies, and a handful of human trials that measured hormone levels as a side finding, not as the main goal. The picture is mixed and incomplete, especially for cortisol, testosterone, and thyroid hormone.

If you take thyroid medication, hormone replacement therapy, testosterone therapy, or corticosteroids, talk with your prescriber before adding CBD. CBD can interact with other drugs, and combining it with hormone medication without medical guidance is not something current research supports.

How CBD Might Touch the Endocrine System

Your endocrine system is the network of glands that make and release hormones, including the adrenal glands (cortisol), the testes (testosterone), and the thyroid gland. According to the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK), an endocrine disease happens when hormone levels are too high, too low, or when the body does not respond to hormones correctly.

The body also has its own cannabinoid system, called the endocannabinoid system, which interacts with glands and hormone pathways. Because CBD affects this system, researchers have started asking whether it also changes hormone output. So far, most of that research has been done in animals or isolated cells, not in large groups of people.

Cortisol and the Stress Response (HPA Axis)

Cortisol is released by the hypothalamic-pituitary-adrenal (HPA) axis, the system that manages your body’s response to stress. A 2024 preclinical study in mice found that daily CBD injections changed how strongly the HPA axis responded to stress, affecting corticosterone (the rodent version of cortisol) release and related stress behaviors in both male and female mice.

This is animal data, not human data. Mice do not process CBD the same way people do, and the doses used in lab studies are often far higher, relative to body weight, than what a person would take in a gummy or tincture. Separately, the National Center for Complementary and Integrative Health (NCCIH) notes that human studies have looked at CBD and anxiety, including a study where people with social anxiety reported less anxiety during a simulated public speaking test after taking CBD compared with a placebo. That study measured self-reported anxiety, not cortisol levels directly.

Testosterone: Mixed and Mostly Preclinical Findings

Research on CBD and testosterone is inconsistent, and most of it comes from animal or lab-dish studies rather than clinical trials in people:

  • A recent rat study comparing THC and CBD found that THC significantly lowered testosterone and raised other reproductive hormones, while CBD showed no significant effect on the same hormones in that experiment.
  • A lab-dish study using a human adrenal cell line found that both THC and CBD reduced androgen (testosterone-related hormone) production at the cellular level, by affecting an enzyme involved in hormone synthesis.
  • An animal toxicity study found that a high daily oral dose of CBD lowered testosterone in the animals studied, though levels stayed within a range considered physiologically normal.
  • A study using human adult testis tissue outside the body (ex vivo) found no direct short-term effect of THC or CBD on testosterone production or germ cells.

Taken together, these studies do not agree with each other, and none of them are large human clinical trials. The National Center for Complementary and Integrative Health lists “male reproductive harm” as one of the effects that has been associated with CBD use, alongside liver injury and drug interactions, though it does not provide dosing or risk thresholds for consumers.

Thyroid Hormones: Limited Human Data

Thyroid research on CBD is also limited. Two examples illustrate the gap between animal and human evidence:

  • A rat study using animals with an induced vitamin D deficiency found that CBD was linked to changes in thyroid-stimulating hormone (TSH) and thyroxine (T4) levels in that specific disease model. This does not mean the same result would happen in a healthy person with no vitamin D deficiency.
  • A registered human clinical trial that gave adults CBD over 29 days and measured TSH as a secondary outcome found a very small average change in TSH between the CBD group and the placebo group, one small enough that it does not point to a meaningful shift in thyroid function from CBD in that trial.

There is no published, well-designed human research showing that CBD improves or worsens thyroid function in people with thyroid conditions like hypothyroidism, hyperthyroidism, Hashimoto’s disease, or Graves’ disease. Anyone managing a thyroid condition should treat “CBD helps thyroid health” claims as unproven.

Evaluating “Hormone Balancer” Marketing Claims

Some product marketing describes CBD as a natural “hormone balancer.” Based on current research, that phrase overstates what is known. Use this checklist when you see a hormone-related CBD claim:

  • Does the claim name a specific study, or is it a general statement with no source?
  • Is the cited study done in humans, or only in animals or cells in a lab dish?
  • Does the study measure the hormone itself, or just a related symptom like mood or sleep?
  • Is the claim about “balancing” hormones, or about a measurable, specific change?
  • Does the product avoid saying CBD can diagnose, treat, support, or prevent any hormone condition?

According to the NCCIH, the FDA has determined that products containing THC or CBD cannot be sold legally as dietary supplements. Any product claiming to treat a specific hormone condition is making a claim current research does not support.

Who Should Use Extra Caution

A few groups should be especially careful and should not use CBD without first talking to a healthcare provider:

  • People taking thyroid medication, such as levothyroxine
  • People on testosterone therapy or other hormone replacement therapy
  • People taking corticosteroids or other medications processed by the liver
  • People who are pregnant, breastfeeding, or trying to conceive
  • People being treated for a diagnosed endocrine disorder

CBD use has been associated with liver injury and drug interactions, so combining it with hormone medications should be discussed with the prescribing provider first, not decided independently.

Frequently Asked Questions

Can CBD raise or lower my cortisol levels?

Animal research suggests CBD can change how the stress-hormone system responds, but this has not been confirmed by human studies measuring cortisol directly. It is not something you can predict or control with over-the-counter CBD products.

Does CBD lower testosterone?

The evidence is mixed. Some lab and animal studies show a drop in testosterone-related hormones with CBD exposure, while others show no effect. There is no reliable human clinical data confirming CBD lowers testosterone in people.

Is CBD safe for people with thyroid disease?
